Low Iron Naturopath


Low iron is one of the most common and most consistently underestimated causes of poor health in women. Fatigue, brain fog, breathlessness, hair loss, poor exercise tolerance, and a general sense of running on empty are all classic signs – yet iron deficiency is frequently missed on standard blood tests, or identified but inadequately treated.

Symptoms of Low Iron

Low iron affects almost every system in the body. Common presentations include:

  • Persistent fatigue and low energy, even after adequate sleep
  • Brain fog, poor concentration, and difficulty with memory
  • Hair thinning or increased hair loss
  • Breathlessness or poor exercise tolerance
  • Pale skin, brittle nails, or cold hands and feet
  • Low mood, irritability, or anxiety
  • Restless legs at night
  • Headaches, dizziness, or heart palpitations in more significant deficiency

One of the challenges with iron deficiency is that many of these symptoms are gradual in onset and easy to normalise – particularly when they develop slowly over months or years. Many of our patients arrive having been told their iron is normal on a standard blood test, yet ferritin (stored iron) is critically low.

Why Standard Testing Often Misses It

A standard GP blood test typically checks haemoglobin – the iron carried in red blood cells. Haemoglobin can remain within range even when ferritin is severely depleted, because the body prioritises keeping red blood cells functional for as long as possible. By the time haemoglobin drops, the body has often been iron deficient at a tissue and storage level for a long time.

We work with your GP to ensure ferritin is tested alongside a full iron studies panel, and we interpret these results in the context of your symptoms – not just in relation to a reference range that was never designed with optimal function in mind.

Common Causes of Low Iron

Heavy or Prolonged Periods

Menstrual blood loss is the most common driver of iron deficiency in women. Heavy periods, frequent bleeding, or cycle irregularities can significantly outpace dietary iron intake. Addressing the hormonal imbalance driving heavy periods – including oestrogen dominance, fibroids, or endometriosis – is an essential part of the longer-term picture, not just supplementing iron in isolation.

Poor Gut Absorption

Iron absorption happens in the small intestine and is highly sensitive to gut health. When the gut is inflamed, the microbiome is imbalanced, or conditions such as coeliac disease or gut dysbiosis are present, iron absorption drops significantly regardless of how much iron you consume or supplement. This is one of the most common reasons iron levels remain low despite supplementation. A comprehensive gut microbiome assessment is often the missing piece in chronic iron deficiency.

Inadequate Dietary Intake

Plant-based diets, restrictive eating, and high-starch or low-protein dietary patterns can all result in insufficient dietary iron. Non-haem iron from plant sources is also significantly less bioavailable than haem iron from animal sources, meaning vegetarians and vegans need to be particularly attentive to both intake and absorption. Our nutritional medicine approach addresses dietary iron through both food-first strategies and supplementation where needed.

Increased Demand

Pregnancy, breastfeeding, rapid growth, and high-intensity exercise all significantly increase the body’s iron requirements. Athletes – particularly endurance athletes – are at elevated risk of iron deficiency due to a combination of increased demand, foot-strike haemolysis, and gastrointestinal losses.

Chronic Inflammation

Chronic low-grade inflammation interferes with iron metabolism through a mechanism called anaemia of chronic disease – the body essentially sequesters iron away from circulation as part of an immune response. In this case, simply supplementing iron is often insufficient without also addressing the underlying inflammatory driver.

Elevated Hepcidin

Hepcidin is a hormone produced by the liver that acts as the body’s primary regulator of iron absorption and distribution. When hepcidin is elevated – which occurs in response to chronic inflammation, infection, or certain chronic diseases – it effectively locks iron inside cells and blocks absorption from the gut, regardless of how much iron you are consuming or supplementing. This is one of the key mechanisms behind anaemia of chronic disease, and it explains why some patients continue to have low circulating iron despite normal or even high iron stores. Identifying and reducing the inflammatory driver behind elevated hepcidin is essential to restoring proper iron metabolism – supplementing iron alone will not overcome a hepcidin block. This is why our approach to persistent iron deficiency always investigates the broader inflammatory and immune picture alongside iron levels themselves.

Thyroid Dysfunction

There is a bidirectional relationship between iron and thyroid function. Low iron impairs thyroid hormone production and conversion, and suboptimal thyroid function can in turn affect iron absorption and metabolism. Assessing both together – rather than treating them as separate issues – is an important part of our approach.

Autoimmune Conditions

Conditions such as coeliac disease, Hashimoto’s thyroiditis, and other autoimmune presentations frequently impair iron absorption or increase iron loss. If iron deficiency is resistant to treatment, an underlying autoimmune condition is always worth investigating as part of the picture.
