Premenstrual syndrome (PMS) refers to a collection of physical and emotional symptoms that occur in the luteal phase of the menstrual cycle – typically in the one to two weeks before menstruation – and resolve within a few days of the period starting. When symptoms are severe enough to significantly impair daily functioning, the condition may be classified as premenstrual dysphoric disorder (PMDD).
PMS is not a personality trait or an inevitable part of being female. It is a physiological response to hormonal imbalance, nutritional deficiencies, gut dysfunction, and nervous system dysregulation – all of which are addressable.
PMS and period pain can present in many ways. Physical symptoms commonly include bloating and fluid retention, breast tenderness, headaches or migraines, cramping and pelvic pain, back pain, fatigue, acne flare-ups around menstruation, constipation or diarrhoea around the period, and heavy or clotted bleeding.
Emotional and psychological symptoms commonly include irritability and mood swings, anxiety, low mood or weepiness, brain fog and difficulty concentrating, food cravings particularly for sugar and carbohydrates, and a general sense of feeling overwhelmed or out of control.

PMS and period pain are most commonly driven by hormonal imbalance – particularly low progesterone in the luteal phase, oestrogen dominance, elevated prostaglandins causing uterine cramping, poor oestrogen clearance through the liver and gut, nutritional deficiencies in magnesium, B6, zinc, and omega-3 fatty acids, chronic inflammation, and dysregulation of the stress response and nervous system.
In some cases, severe period pain is a sign of an underlying condition such as endometriosis or adenomyosis, which requires investigation. Our naturopaths will always consider these possibilities during assessment and refer appropriately where needed.
The gut plays a significant and often overlooked role in PMS. The gut microbiome is directly involved in oestrogen metabolism through the estrobolome – a group of gut bacteria that regulate how oestrogen is processed and eliminated from the body. When the gut microbiome is disrupted, oestrogen that should be cleared is instead reactivated and recirculated, worsening oestrogen dominance and amplifying PMS symptoms.
Poor gut health also affects serotonin production – approximately 90 percent of the body’s serotonin is made in the gut. Low serotonin in the luteal phase is a key driver of mood-related PMS symptoms including anxiety, irritability, and low mood. Conditions such as IBS, food sensitivities, leaky gut, and SIBO all worsen hormonal PMS by interfering with oestrogen clearance, serotonin production, and systemic inflammation.
