In a healthy circadian rhythm, cortisol is high in the morning to promote waking and alertness, and drops progressively throughout the day, reaching its lowest point at night to allow sleep onset. When the stress response is chronically activated, this rhythm becomes disrupted – cortisol can remain elevated into the evening, making it difficult to wind down, or can spike in the early hours of the morning causing that characteristic 3am waking with a racing mind. Progesterone has a natural calming, sleep-promoting effect through its influence on GABA receptors. When progesterone drops – as it does in the luteal phase of the menstrual cycle, during perimenopause, and after childbirth – sleep disruption often follows. Oestrogen fluctuations contribute to night sweats and temperature dysregulation that fragment sleep. Thyroid dysfunction – both overactive and underactive – significantly affects sleep quality. Blood sugar drops during the night are a very common and frequently missed cause of early morning waking. When glucose drops in the early hours, the body releases adrenaline and cortisol to raise it – which is exactly what wakes you up at 3 or 4am feeling alert or anxious. Stabilising blood sugar through diet and targeted supplementation can resolve this pattern quickly for many patients.
Melatonin is the hormone that signals to the body that it is time to sleep. Its production is heavily influenced by light exposure – suppressed by blue light from screens in the evening, and triggered by darkness. Melatonin production also declines with age and can be impaired by nutrient deficiencies, particularly magnesium and B6, which are required as cofactors in its synthesis.
The gut microbiome influences sleep through several pathways – serotonin production (serotonin is a precursor to melatonin), inflammatory signalling, and the vagal nerve connection between the gut and brain. An overactivated nervous system – one that struggles to shift out of sympathetic dominance (fight or flight) into the parasympathetic state (rest and digest) – makes sleep onset and maintenance genuinely difficult. Magnesium is perhaps the single most important nutrient for sleep – it supports GABA activity, muscle relaxation, and nervous system downregulation. Deficiency is extremely common and directly associated with poor sleep, restless legs, and difficulty switching off. B6 is required for melatonin and serotonin synthesis. Iron deficiency is associated with restless legs syndrome – a frequently missed cause of sleep disruption. Zinc supports melatonin regulation.
