Helicobacter pylori (H. pylori) is one of the most common bacterial infections in the world – and one of the most frequently overlooked causes of chronic digestive symptoms. It is estimated that over half the global population carries H. pylori, yet many people have no idea it is present until symptoms become hard to ignore.

What Is H. Pylori?

Helicobacter pylori is a spiral-shaped bacterium that colonises the stomach lining. Unlike most bacteria, it is able to survive in the highly acidic environment of the stomach by producing an enzyme called urease, which neutralises stomach acid in its immediate environment and allows it to burrow into the gastric mucosa.

Once established, H. pylori triggers chronic inflammation of the stomach lining, disrupts normal stomach acid production, and can significantly impair digestive function over time. It is the leading cause of chronic gastritis, peptic ulcers, and is classified as a carcinogen due to its association with gastric cancer when left untreated long-term.

H. pylori is transmitted person to person – most commonly via oral contact, contaminated water or food, or close household contact. Many people acquire it in childhood.

Symptoms of H. Pylori Infection

H. pylori does not always cause obvious symptoms, particularly in the early stages. When symptoms do occur, they often include:

  • burning or gnawing pain in the upper abdomen, often worse when the stomach is empty
  • persistent nausea, particularly in the morning
  • bloating and fullness after small amounts of food
  • frequent burping
  • reflux and heartburn
  • loss of appetite
  • unintentional weight loss
  • fatigue and low energy
  • dark or tarry stools (if ulceration is present – seek medical attention promptly)

Because these symptoms overlap with many other digestive conditions including IBSSIBO, and general digestive dysfunction, H. pylori is often missed without specific testing.

H. Pylori Testing

Testing for H. pylori is straightforward and non-invasive. Options include:

  • stool antigen test – a highly accurate and commonly used first-line test
  • urea breath test – also very accurate, performed at a pathology centre
  • blood serology – detects antibodies to H. pylori, though cannot distinguish active from past infection
  • biopsy via endoscopy – used when endoscopy is being performed for other reasons

It is important to note that antibiotics and proton pump inhibitors (PPIs) should be avoided for at least two weeks before testing, as they can produce false negative results.

Conventional Treatment and Where Naturopathy Fits In

The standard medical treatment for H. pylori is a course of triple therapy – two antibiotics combined with a proton pump inhibitor, typically taken for 7-14 days. Eradication rates with triple therapy vary, and treatment failure due to antibiotic resistance is increasingly common.

Naturopathic support is valuable both alongside conventional treatment and in cases where natural approaches are preferred or where standard treatment has been unsuccessful. Our role is not to replace medical treatment where it is appropriate, but to provide additional and complementary support that may improve outcomes and aid recovery.
