The thyroid is a small gland with an enormous influence over how the body functions. It regulates metabolism, energy production, mood, weight, temperature, heart rate, digestion, hormonal balance, and cognitive function – and when it is not working properly, almost everything suffers. Despite this, thyroid dysfunction is one of the most commonly missed and undertreated conditions in conventional medicine, particularly when blood results fall within a “normal” reference range that many clinicians agree is too broad.

Hashimoto’s is an autoimmune condition in which the immune system attacks the thyroid gland, progressively impairing its ability to produce sufficient thyroid hormone. It is the most common cause of hypothyroidism in Australia and is frequently either undiagnosed or diagnosed but not properly managed beyond a prescription for thyroxine. Hashimoto’s has a strong connection to gut health – intestinal permeability and gut dysbiosis are both well-established drivers of autoimmune thyroid activity. An underactive thyroid slows virtually every metabolic process in the body. Symptoms typically develop gradually and are easy to dismiss or attribute to other causes – fatigue, weight gain, brain fog, constipation, cold sensitivity, dry skin, hair thinning, low mood, and poor memory are among the most common. Many patients with clear hypothyroid symptoms are told their levels are normal on a standard TSH test. We look at the full thyroid picture, including free T3, free T4, and thyroid antibodies, and we treat symptoms as well as numbers.
Subclinical hypothyroidism describes a pattern where TSH is elevated but T3 and T4 remain within the normal range. Conventional medicine often takes a watchful waiting approach, but for many patients this comes with significant, real symptoms – particularly fatigue, weight difficulty, and brain fog. Nutritional and herbal medicine can be highly effective in supporting thyroid function at this stage, often preventing progression to overt hypothyroidism.
An overactive thyroid drives the body into a state of accelerated function – rapid heartbeat, unexplained weight loss, excessive sweating, insomnia, anxiety, irritability, and in some cases changes to vision or eye prominence (in Graves’ disease specifically). The thyroid and reproductive hormones are closely interconnected. Suboptimal thyroid function can disrupt the menstrual cycle, contribute to oestrogen dominance, impair progesterone production, and worsen PMS, PCOS, and perimenopausal symptoms. A standard GP blood test typically measures only TSH – thyroid stimulating hormone. TSH is a signal from the pituitary gland telling the thyroid to produce more hormone. It is a useful marker but it does not tell us how much active thyroid hormone is actually circulating, how well T4 is being converted to the active T3 form, or whether thyroid antibodies are present indicating an autoimmune process.
We request or interpret comprehensive thyroid panels that include free T3, free T4, TSH, and thyroid antibodies (anti-TPO and anti-thyroglobulin). This gives a complete picture of how the thyroid system is actually functioning, and it is not uncommon to find significant clinical findings on a full panel that would have been missed on TSH alone.
The thyroid requires iodine, selenium, zinc, iron, and tyrosine to produce and convert thyroid hormone. Deficiency in any of these – which is common, particularly selenium and zinc – directly impairs thyroid function. Our nutritional medicine approach identifies and corrects these deficiencies based on testing rather than assumption.
Approximately 20% of T4 to T3 conversion occurs in the gut. A dysbiotic or inflamed gut therefore directly impairs thyroid hormone availability regardless of how well the thyroid gland itself is functioning. There is also a well-established link between intestinal permeability and autoimmune thyroid conditions – the gut is frequently the origin of the immune trigger driving Hashimoto’s. Gut microbiome testing is an important tool in our thyroid investigation protocol.
Elevated cortisol suppresses TSH output and impairs T4 to T3 conversion. Chronic stress is one of the most significant and underappreciated drivers of thyroid dysfunction, and it is also one of the factors most likely to be overlooked in conventional thyroid management. Systemic inflammation suppresses thyroid function through multiple mechanisms – including reducing TSH output, impairing T4 to T3 conversion, and driving autoimmune activity.
