Hormonal weight gain has a distinct quality to it. It tends to accumulate in specific places – the abdomen, hips, and thighs – and it resists the usual approaches. Eating less and exercising more may produce little to no result when hormones are driving the problem, because the underlying issue is not simply caloric – it is biochemical.
At The Naturopathic Co., we work with patients every day who are frustrated by weight that will not shift despite their best efforts. For many of them, hormonal imbalance is a significant piece of the puzzle – and once we address the hormonal drivers, weight management becomes considerably more achievable.
We offer in-person consultations in Melbourne and telehealth appointments Australia-wide, including Sydney and Brisbane.
Hormones regulate almost every aspect of metabolism – how efficiently you burn fuel, where fat is stored, how hungry you feel, how your body responds to stress, and how well you sleep. When any of these hormonal systems fall out of balance, weight gain can follow – and it often does so in ways that feel completely disconnected from diet and exercise habits.
The most commonly implicated hormones in weight gain include oestrogen, progesterone, cortisol, insulin, thyroid hormones, and leptin. These do not operate in isolation – imbalances in one hormone almost always affect the others, which is why a whole-system approach is so important.
You can read more about our broader approach to hormonal imbalance here.
Oestrogen dominance – where oestrogen is elevated relative to progesterone – is one of the most common hormonal contributors to weight gain in women. It tends to promote fat storage particularly around the hips, thighs, and abdomen, and is often accompanied by bloating, fluid retention, PMS, breast tenderness, and heavy or irregular periods.
Oestrogen is metabolised primarily through the liver and the gut, meaning that poor liver detoxification, gut dysbiosis, and microbiome imbalance can all contribute to oestrogen accumulating in the body rather than being cleared efficiently. This is why gut health support is often an important part of addressing oestrogen-driven weight gain.
Cortisol is the body’s primary stress hormone, and chronically elevated cortisol is one of the most significant drivers of abdominal fat accumulation. When the body is under prolonged stress – whether physical, emotional, or lifestyle-related – elevated cortisol promotes the breakdown of muscle tissue, increases appetite and cravings for high-calorie foods, drives blood sugar dysregulation, and directs fat storage toward the abdominal region.
Many patients with stress-related weight gain also experience disrupted sleep, persistent fatigue, anxiety, and mood changes alongside the weight concern. Addressing stress and nervous system dysregulation is a core part of our approach for these patients.
The hormonal shifts of perimenopause and menopause are among the most common triggers for significant and sudden weight changes – particularly increased abdominal fat accumulation – even in women who have never previously struggled with their weight.
Declining oestrogen and progesterone affect insulin sensitivity, fat distribution, metabolism, sleep quality, and muscle mass – all of which contribute to weight gain. Many women also find that the dietary and exercise strategies that worked for them in their thirties simply stop being effective during this hormonal transition.
Naturopathic and nutritional support during perimenopause and menopause can help address these hormonal shifts, support metabolic health, and make weight management more achievable. Read more about our approach to hormonal health.
Polycystic ovarian syndrome (PCOS) is strongly associated with weight gain and difficulty losing weight, driven by a combination of insulin resistance, elevated androgens, and hormonal dysregulation. Weight management in PCOS requires a specific and targeted approach that addresses the underlying hormonal and metabolic drivers rather than simply restricting calories.

An underactive thyroid – or even suboptimal thyroid function that does not reach the threshold for a formal diagnosis – can significantly slow metabolism and contribute to weight gain, fluid retention, and an inability to lose weight despite dietary changes. Leptin is the hormone responsible for signalling to the brain that the body has had enough to eat. In leptin resistance – which commonly develops alongside obesity, insulin resistance, and chronic inflammation – this signalling breaks down. The brain effectively stops receiving the “full” message, driving ongoing hunger, overeating, and fat storage regardless of how much food has been consumed.
Addressing leptin resistance involves improving insulin sensitivity, reducing systemic inflammation, supporting gut health, improving sleep, and managing stress – all areas where naturopathic medicine can provide meaningful support.
At The Naturopathic Co., we begin by understanding your individual hormonal picture through thorough case assessment and targeted functional testing. From there, support may include:
