Metabolism refers to the sum of all the chemical processes the body uses to convert food into energy. Basal metabolic rate (BMR) – the energy the body burns at rest – accounts for the majority of daily calorie expenditure for most people. When this rate slows, the body requires less energy to function, making weight gain easier and weight loss considerably harder.

The thyroid is the master regulator of metabolism. Even subtle or subclinical thyroid dysfunction – including impaired T4 to T3 conversion and autoimmune thyroid conditions such as Hashimoto’s – can significantly slow metabolic rate. When cells become resistant to insulin, the body struggles to use glucose efficiently as a fuel source. This drives fat storage, energy fluctuations, sugar cravings, and metabolic dysfunction. Prolonged stress keeps cortisol elevated, which over time promotes muscle breakdown, increases fat storage – particularly around the abdomen – and impairs the metabolic function of other hormones including thyroid and insulin. One of the most counterproductive contributors to a slow metabolism is a history of chronic calorie restriction. When the body is repeatedly deprived of adequate fuel, it adapts by downregulating metabolic rate to conserve energy – a process sometimes called metabolic adaptation. This can make weight loss increasingly difficult over time despite eating very little, and is something we see regularly in patients with a long dieting history.
Sleep is when the body repairs, regulates hormones, and resets metabolic function. Chronic poor sleep disrupts leptin and ghrelin – the hunger and satiety hormones – increases cortisol, worsens insulin resistance, and significantly impairs metabolic efficiency. Addressing sleep is often a non-negotiable part of metabolic support.
The gut microbiome plays a significant and increasingly well-understood role in metabolism. Dysbiosis – imbalance in the gut bacterial community – has been linked to impaired energy extraction from food, increased fat storage, insulin resistance, and systemic inflammation that further disrupts metabolic function.
