ADHD (attention deficit hyperactivity disorder) is a neurodevelopmental condition characterised by difficulties with attention, impulse control, and in some presentations, hyperactivity. It is more common than previously recognised – particularly in adults, and particularly in women, who are significantly underdiagnosed due to differences in how ADHD presents across genders.
What is increasingly understood is that ADHD is not simply a behavioural or psychological condition – it involves measurable differences in dopamine and noradrenaline signalling, prefrontal cortex activity, gut microbiome composition, inflammatory status, and nutritional cofactors. All of these are areas where naturopathic and nutritional medicine can provide meaningful support.
ADHD is fundamentally associated with dysregulation of dopamine – the neurotransmitter responsible for motivation, reward, focus, and follow-through. Dopamine synthesis depends on specific nutritional cofactors including tyrosine, iron, B6, folate, and zinc. Deficiencies in any of these directly impair the brain’s ability to produce and regulate dopamine. Supporting the nutritional foundations of neurotransmitter synthesis is a central part of our approach.
Iron is a critical cofactor in dopamine synthesis, and iron deficiency – even in the absence of frank anaemia – is one of the most well-researched nutritional contributors to ADHD symptoms. Studies consistently show lower ferritin levels in children and adults with ADHD compared to controls, and iron supplementation has demonstrated meaningful improvements in attention and hyperactivity in iron-deficient patients. Iron status is one of the first things we investigate in patients presenting with focus and attention concerns.
Zinc plays a key role in dopamine metabolism and regulation of the dopamine transporter. Zinc deficiency is associated with increased ADHD symptom severity, and zinc supplementation has shown benefit in several clinical trials. Magnesium deficiency – extremely common in the general population – is associated with hyperactivity, restlessness, poor sleep, and difficulty concentrating. Both are frequently low in patients with ADHD presentations.
The brain is approximately 60 percent fat, and omega-3 fatty acids – particularly DHA and EPA – are essential structural and functional components of brain cell membranes and neurotransmitter receptor sites. Omega-3 deficiency is associated with reduced attention, increased impulsivity, and poorer cognitive performance. The evidence base for omega-3 supplementation in ADHD is among the strongest of any nutritional intervention in this area.
The ADHD brain is particularly sensitive to blood sugar fluctuations. Rapid drops in blood glucose trigger adrenaline release, worsening hyperactivity, impulsivity, and irritability. Many patients find that stabilising blood sugar through dietary changes and targeted supplementation produces a noticeable improvement in focus and emotional regulation. Emerging research is revealing a strong connection between gut microbiome composition and ADHD. Children and adults with ADHD show distinct patterns of gut dysbiosis compared to neurotypical controls, and the gut-brain axis directly influences dopamine and serotonin production, neuroinflammation, and cognitive function. Chronic low-grade inflammation impairs prefrontal cortex function – the area of the brain most responsible for attention, planning, impulse control, and executive function. Reducing neuroinflammatory drivers through diet, gut health support, and targeted anti-inflammatory nutrients can meaningfully improve cognitive performance and emotional regulation.
Sleep problems are extremely common in patients with ADHD – both as a consequence of the condition and as a driver of worsened symptoms. Poor sleep significantly impairs prefrontal cortex function, worsens impulsivity and emotional dysregulation, and reduces dopamine receptor sensitivity. Elevated cortisol from chronic stress directly impairs prefrontal cortex function and dopamine signalling – effectively producing or worsening ADHD-like symptoms in people who may not have the underlying neurological profile. Oestrogen supports dopamine activity in the prefrontal cortex, which is why many women notice a significant worsening of ADHD symptoms during the luteal phase of their cycle, during perimenopause, and postnatally – when oestrogen drops.
