Endometriosis is an inflammatory disease in which tissue similar to the lining of the uterus grows outside the uterus – on the ovaries, fallopian tubes, bowel, bladder, and other pelvic structures. Unlike the uterine lining, this tissue has no way to leave the body, causing inflammation, scarring, and the formation of adhesions and cysts known as endometriomas.
It is important to understand that endometriosis is not simply a hormonal condition – it is primarily an inflammatory and immune disease that is significantly influenced by hormones, particularly oestrogen. This distinction matters because treatment must target inflammation and immune dysregulation, not just hormones.
Endometriosis can present very differently from person to person. Some women experience severe, debilitating pain while others have minimal symptoms despite significant disease. Common presentations include painful periods that worsen over time, pelvic pain outside of menstruation, pain during or after intercourse, pain with bowel movements or urination particularly around the period, heavy or irregular bleeding, bloating and digestive disturbance, fatigue and low energy, and difficulty conceiving.

A disrupted gut microbiome impairs oestrogen clearance through the estrobolome – a collection of gut bacteria responsible for metabolising and eliminating oestrogen. When this process is compromised, oestrogen is reactivated and recirculated rather than eliminated, driving the oestrogen dominance that feeds endometriosis lesions and worsens inflammation.
Leaky gut is also highly relevant in endometriosis, as increased intestinal permeability allows bacterial endotoxins to enter the bloodstream and amplify the systemic immune activation that drives the disease.
