Oestrogen dominance occurs when oestrogen levels are disproportionately high relative to progesterone. This can happen in two ways – oestrogen can be genuinely elevated, or progesterone can be low while oestrogen remains within normal range. In both cases, the result is the same: an oestrogen-to-progesterone ratio that is out of balance and producing symptoms.
Oestrogen dominance is extremely common in the modern world. Environmental oestrogens found in plastics, pesticides, personal care products, and food packaging – known as xenoestrogens – add to the body’s oestrogenic load. The most important and most overlooked drivers of oestrogen dominance are poor gut health and impaired liver function. Understanding this connection is key to treating oestrogen dominance effectively.
Oestrogen is processed by the liver and then passed into the gut for elimination. In the gut, a collection of bacteria known as the estrobolome are responsible for metabolising oestrogen so that it can be excreted from the body. When the gut microbiome is disrupted – through dysbiosis, leaky gut, constipation, or SIBO – an enzyme called beta-glucuronidase becomes overactive. This enzyme deconjugates oestrogen that has already been processed for elimination, reactivating it and allowing it to be reabsorbed into the bloodstream rather than excreted.

The liver is equally central. Phase one and phase two liver detoxification pathways are responsible for breaking oestrogen down into safer metabolites before it reaches the gut. When liver function is impaired – through poor diet, alcohol, nutrient deficiencies, or toxic load – oestrogen is not properly metabolised and accumulates in the body.
